Overview

An AED is a small, portable device that evaluates the heart's rhythm and prompts the rescuer to administer an electrical shock if one is required. AEDs are designed for use by lay responders and require minimal training; they guide the user through each step of the defibrillation process by providing voice and/or visual prompts.

Facts about Sudden Cardiac Arrest

Sudden cardiac arrest (SCA) is the number one killer in Canada: Up to 40,000 Canadians die each year from it, and it can strike anyone, anywhere. SCA is a condition in which the heart stops beating due to a malfunction in the heart's electrical system. The technical term for this malfunction is ventricular fibrillation (VF). When in VF, the heart is unable to pump blood efficiently-it stops beating. When that happens, the victim quickly loses consciousness and stops breathing.

SCA can be caused by drowning, electrocution, suffocation, a lightning strike, hypothermia or a massive heart attack (a blockage in an artery that supplies blood to the heart). The only way to save someone in SCA is to deliver an electrical shock using a defibrillator. Without immediate treatment from a defibrillator, 90% to 95% of SCA victims die.

Emergency medical service (EMS) professionals and firefighters save many people each year, but time delays keep them from saving many more. In cases of cardiac arrest, for example, help often arrives too late. In major Canadian centres, EMS typically arrives 9-10 minutes after the call to 911. And yet brain damage can start to occur in 4 to 6 minutes. Studies have shown that with every minute that passes without defibrillation, a victim's chance of survival is reduced by 10%.

But there is good news: A recent major study on public-access defibrillators found that when an AED is used on a casualty within the first three minutes, the survival rate is as high as 70%. In short, thousands of deaths could be prevented by providing quick and easy access to defibrillation.

How does an AED work?

Adhesive pads attached to wires that connect to the AED are placed on the patient's chest, and the machine takes the responder through the process with voice prompts. The AED delivers an electrical shock through the body to the heart, if it is needed.

The AED is one vital link in what the Heart and Stroke Foundation calls the "Chain of Survival." First, someone must recognize that there is an emergency and call for help. While waiting for help to arrive CPR must be started. As soon as an AED is available, it can be used. The fourth step is advanced medical care.

Who can use an AED?

Just about anyone. No medical background is needed and all it takes is four hours of training-less if the AED instruction is part of a first aid or CPR course. MediStat provides a number of course options to choose from.

The American Heart Association says: "AEDs are sophisticated, computerized devices that are reliable and simple to operate, enabling lay rescuers with minimal training to administer this lifesaving intervention." Flight attendants, security guards, lifeguards, family members and many other trained laypersons have used AEDs successfully.

Community health workers and emergency response professionals of all levels are typically trained to use an AED, but trainees could also include volunteers in the community who are interested, willing to learn and willing to participate in emergency response. The more people who know how to use AEDs, the more effective they become in saving lives.

Why should we purchase an AED?

The best way to reduce deaths from sudden cardiac arrest is to have an AED on site-whether it's an office building, sports arena, community centre, or your own home-and then ensure that potential responders are trained to use it. It makes sense: an AED costs about the same as a well-equipped computer-a minor expense when you consider it may save a life.

That is why governments and organizations such as the Heart and Stroke Foundation and Canadian Red Cross endorse the widespread deployment of AEDs. In the U.S., the American Heart Association has stated that early defibrillation could save as many as 50,000 lives each year. By extrapolation, that suggests up to 5,000 could be saved in Canada.

What about training?

Training is critical for AED deployment to be effective. MediStat is an authorized provider of the Canadian Red Cross, the leading government-recognized first aid training agency in Canada. Are there liability concerns?

AEDs are designed to reduce the risk of operator misuse. For example, a shock will not be administered if the defibrillator does not detect a heart rhythm that needs defibrillating. Using an AED is no different from performing other medical acts or first aid (which are covered by Good Samaritan laws): If AED operators perform as they are trained to perform, they are protected. No one in Canada has ever been sued for using an AED.

To minimize the liability risk, the Heart and Stoke Foundation recommends that organizations ensure operators have medical oversight, are properly trained and that protocols for continued training, operation and equipment maintenance are in place.
